 How do you access palliative services for your clients?

The Common Referral Form makes communication easier for people needing palliative care.  They won't have to "tell their story" over and over again.  The Common Referral Form was developed collaboratively by health care organizations across the Toronto Central, Central, Central East, Central West and Mississauga Halton LHINs.

A completed Common Referral Form contains comprehensive information for new providers becoming involved with a client and fosters communication across multiple care providers.  One of the great attributes of the Common Referral Form is that the same form can be used to make multiple referrals for a client to different service providers across the continuum of care.
