What is the consultation aspect of the PPSMC service?
This service is to assist you, the health care provider, address your client's palliative or end-of-life care needs.
We know you want to effectively manage your resident/patient/client's pain or symptoms. When you identify that additional coaching/mentoring would be helpful, we are here to assist you by phone, email, or in person. For example, your assessment tells you that Mrs. Smith's pain is not well controlled. You are unsure what to do or how to report to her physician to get new orders. We can offer you coaching on the next steps to get Mrs. Smith comfortable. Or, Mr. Doe has dementia and he is not able to express his pain to you. We can offer steps to help you better assess his pain to ensure that you can manage his pain or other symptoms to make sure he is comfortable.
Please note: As part of this coaching activity with a health care provider, a client/patient may be present. The health care provider remains the one responsible for care.
