Consultants

Jennifer Land, Consultant - RN, BScN, CHPCN (C)

Jennifer has worked as a Palliative Pain and Symptom Management Consultant since August 2009. Prior to assuming this position she was a Care Coordinator with the Dorothy Ley Hospice Community Program for 10 years. Jennifer is nationally certified in Hospice Palliative Care.

Tracey Human, Consultant - RN

Tracey is a seasoned professional with 20 years of Palliative Care expertise.  Her 30 years of nursing encompassed high acuity units to community care, along with 13 years in the business sector delivering consultation on business development, programming, operational analysis, marketing and training.  Traceyis currently pursuing the CHPCN(C) designation and Masters Certificate in Healthcare Management. 

Brenda Caldwell, Consultant - RN BScN CON(C) CHPCN(C)

Brenda Caldwell joined PPSMC Program after working in the Residential Program at Dorothy Ley Hospice in 2009-2010.  She worked for Interlink Community Cancer Nurses from 1991-2005 and worked as a Community Interlink nurse at Princess Margaret Hospital from 2005-2009.  She has specialty nursing certification in both oncology and palliative care nursing. She has a passion for mentoring, and building strong relationships within the Palliative Care community.
